Claim forms are available, and filed, in the City Clerk's Office at 214 E. Branch Street. You may also find a claim form online which can be printed.
Filing must take place within 6 months or 182 days, whichever is longer, from the accrual of the cause of action. Real property claims must take place within one year.
A claim form must be filed with an original signature. Estimates, receipts, or photographs should be submitted with the claim. Claims are forwarded to the City’s claims administrator for processing.
